Condom Dispenser Application

Condom dispensers hold 50 condoms at one time and allow free access to condoms in spaces such as community organisations and public toilets.

Terms of agreement:

I understand and acknowledge all terms of agreement outlined below in relation to receiving supply, installation, and initial stock of condom vending machines/dispensers purchased with WA Department of Health funds:

1. Only limited WA Department of Health funds are available and therefore ongoing supply of condoms and maintenance of the condom vending machines/dispensers is not possible.
2. The Organisation has obtained all the required approvals from relevant authorities.
3. The condom vending machines/dispensers have been provided for the Organisation to make condom stock available at the described location of the organisation.
4. The ordering and restocking of condoms and the condom vending machine/dispenser maintenance is the responsibility of the Organisation.
5. The installation costs of the condom vending machines/dispensers will be funded by the WA Department of Health.
6. The location of condom vending machine/dispensers may be added to the Find Free Condoms feature on the Get the Facts website.
